Ghid de utilizare a investigatiilor radiologice si imagistice la copil
Grupul de Radiologie Pediatrica (GRP) a fost implicat in realizarea celei de-a doua editii (2019) a Ghidului de utilizare a investigatiilor radiologice si imagistice, sub egida Societatii de Radiologie (SRIM). Ghidul a fost aprobat, in integralitatea sa, de catre Comitetul National al SRIM si urmeaza sa fie avansat de Consiliul Director al SRIM, catre Ministerul Sanatatii, in vederea aprobarii prin Ordin de ministru.
Pana la oficializarea legislativa a acestui Ghid, GRP va pune la dispozitie capitolul de radiologie pediatrica, aprobat de Comitetul GRP ca document de referinta, sustinut si recomandat de catre GRP. Publicarea pe site este realizata cu acordul autorilor capitolului.

Radiologia pediatrica si CoViD-19 / SARS-CoV2
(Aducere la zi: 05 iulie 2021)
Pandemia SARS-CoV2 ne obliga la adaptare si organizare: Ce facem?, Cand facem?, Cum facem?
Daca inainte trebuia evaluat beneficiul asteptat al unei investigatii, prin raportare la riscul de iradiere, riscurile substantelor de contrast si ale sedarii, acum a aparut mult mai pregnant si riscul biologic. Acesta include riscul infectarii personalului din radiologie, al altor lucratori din spital, dar si al pacientilor (fiei ei CoViD-19 negativi sau la risc/”suspecti” si care se vor dovedi in final negativi).
Am constatat ca investigarea pacientilor cu contagiozitate crescuta (si in particular copii) este o intreprindere foarte dificila, cel putin intr-un spital fara experienta in boli cu contagiozitate crescuta, de aceea o impartasim. Am pornit de la traducerea-adaptarea procedurilor pentru RGmobil si CT (facuta de dr. Marian POP -Targu Mures) emise de BSTI (British Society for Thoracic Imaging) si le-am adaptat situatiei de la Spitalul “Grigore Alexandrescu” din Bucuresti, tinand cont si de documentele oficiale privind pregatirea pentru epidemie (primite de la MS si CSSU), de planul spitalului intocmit in acest sens, de opinia medicului epidemiolog si de experienta practica a medicilor radiologi si asistentilor de radiologie din sectie. Va prezentam solutiile adoptate la Spitalul “Grigore Alexandrescu”, care este un spital de nivel III de pediatrie, de urgenta, avand aprope toate specialitatile dar fara sectie de boli infectioase si cu structura multi-pavilionara. Spitalul a fost desemnat sa primeasca pacienti pediatrici cu patologie din aproape toate specialitatile si in particular cazuri de urgenta, indiferent daca sunt “negative”, “pozitive” sau “suspecte” in privinta infectiei cu CoViD-19. In prezent nu suntem desemnati pentru tratamentul pneumoniei cu CoViD-19 ca patologie unica sau determinanta. Tinand cont de conditiile locale, spitalul a organizat mai multe puncte de triaj epidemiologic: La UPU, la Camera de Garda Ortopedie si la accesul in alte pavilioane separate. Un pavilion separat a fost organizat pentru pacientii “suspecti” sau “pozitivi” (cu patologie, pediatrica, chirurgicala, ortopedica), inclusiv cu ATI si bloc operator. La UPU (situat in cladire alaturata) exista camera de decontaminare si salon de izolare, utilizabile pentru astfel de pacienti; a fost marit si numarul de saloane cu un singur pat, in vederea minimizarii riscurilor de contaminare intre pacienti. Am avut posibilitatea sa dedicam cate un aparat mobil digital de radiografie, cat si cate un ecograf, atat in pavilionul dedicat pentru pacienti “pozitivi”, cat si in zona de izolare a UPU.
In etapa de transmitere comunitara, strategia a fost re-adaptata, orice pacient putand fi infectat, chiar daca cu un risc mai mic. Au fost astfel introduse masuri suplimentare de protectie chiar si in cazul pacientilor “obisnuiti” (care nu se incadreaza in definitia de caz “suspect” sau “confirmat”).
Procedurile operationale de realizare a radiografiilor, ecografiilor si computer-tomografiilor (anexate) au tinut cont de aceste organizari. Speram sa fie de folos colegilor din alte spitale, prin adaptare la conditiile locale.
In literatura de specialitate au fost propuse si solutii de expunere pentru radiografii cu aparatul mobil, din alta camera, prin ecran de sticla (vezi mai jos)
In alta ordine de idei, am facut o sinteza a recomandarilor aparute in utilizarea imagisticii pulmonare pentru infectiile respiratorii cu CoviD-19 la copil, relevandu-se o atitudine diferita fata de cea abordata la pacientii adulti (imagistica mult mai putin recomandata). SFIPP (Societatea Francofona de Imagistica Pediatrica) a elaborat un ghid foarte coerent. ESPR a elaborat mai de curand un ghid, chiar ceva mai “restrictiv” decat cel al SFIPP. Alte societati internationale au facut doar mentiuni tangentiale (ACR) sau nu au formulat inca recomandari proprii (SPR). Atasez protocolul adoptat la spitalul nostru, precum si Ghidurile SFIPP si ESPR. Atasez de asemenea un articol privind utilizarea CT la copii, in prima parte a epidemiei, in China si un studiu privind imagistica in cazurile pediatrice de CoViD19, aparut sub egida ESPR..
Investigatia imagistica pentru alte patologii, la pacientii “pozitivi” CoViD-19, va urmari ghidurile existente pentru patologiile respective, insa cu o mai atenta evaluare -atat a necesitatii cat si a riscurilor. Editia 2019 a Ghidului de utilizare a investigatiilor radiologice si imagistice medicale, finalizata de catre Comitetul editorial si coordonatorii de capitole si mai apoi aprobata de catre Comitetul National al SRIM, este in asteptarea deciziei Consiliului Director in vederea publicarii si transformarii in act normativ, pe canale oficiale (actualizarea Ordinului MS Nr. 1393/2010).
In contextul epidemic SARS-CoV-2, Ministerul Sanatatii a publicat o serie de Ghiduri ale Comisiilor de Specialitate. Printre acestea remarc Recomandarile Comisiei de Alergologie si Imunologie clinica, care face referire la utilizarea substantelor de contrast, cu afirmatii clare si ne-echivoce, avand aplicabilitate generala, nu doar in contextul pandemiei! ESSENTIALS IN PAEDIATRIC RADIOLOGY (LEVEL I-II) – 4th Edition
TRAINING FOR CENTRAL (AND FORMER EASTERN) EUROPE
This course is organized by the Paediatric Radiology Group of the Romanian Radiological Society (GRP-SRIM), with the help of the Division of Pediatric Radiology, Department of Radiology, Medical University of Graz (MUG) / Austria and under the patronage of the Medical University “Victor Babes” Timisoara / Romania. Previous editions were held in Cluj-Napoca (2016), Bucharest (2017) and Brasov (2019). This edition was first scheduled for 2020 but postponed two times by one year, because of the CoViD19 Pandemic.
The European Society of Paediatric Radiology (ESPR) included the course in its outreach programme and is it’s main sponsor.
The Emergency Children’s Hospital “Louis Turcanu” of Timisoara (Romania) is hosting this fourth edition of the course.
After two years, an „Advanced” (Level III) course is planned, for a selection of participants that will have completed previous „Essentials” courses. The first edition of the Level III course was held in Graz (2018).
The Romanian College of Physicians (CMR) approved 24 CME credits for this course (Nr.3839/12.05.2022).
Course language: English.
Course schedule: Morning Hands-on (participation in clinical routine, in small groups); Morning lectures (the methods and their paediatric applications will be presented – Radiography and Fluoroscopy, Ultrasound, CT, MRI and basics for interventions); Afternoon lectures (system and age oriented, specific paediatric challenges -NAI, trauma imaging, etc.); Afternoon workshops (case-based interactive session in small groups, focusing on symptom oriented imaging). Focus countries – Romania and other countries in the region: Albania, Belarus, Bosnia-Herzegovina, Bulgaria, Estonia, Kosovo, Latvia, Lithuania, North Macedonia, Moldova, Montenegro, Serbia, Slovakia, Ukraine. Eligibility criteria as well as the order of application will be taken into account to select the applicants.
Application/Selection criteria: Maximum 32 participants (about half from Romania and half from the other countries listed above). Young radiologists (first years after certification) or last-year residents, wanting to commit themselves (in a significant proportion) to pediatric radiology, employed (under a long-time or unlimited contract) in a big hospital (county, municipal, or mono-specialty) having at least 1-2 pediatric clinical departments and at least 3-4 radiologists, but no dedicated pediatric radiology. Applicants from areas with the least pediatric radiology service and training will be given priority. First came first served, if criteria do not allow a ranking of applicants. Applicants from Romania are eligible only if they are GRP members! Register now to become a GRP member!
Course registration fee: 200 E. ESPR supports a reduced fee for radiologists in training (proof requested): 100 E. The fee covers the course logistics, the coffee breaks and lunches. Teaching activities and Syllabus will be granted by the ESPR.
Romanian participants will pay in local currency (1000/500RON) – only GRP members are eligible.
You have to make the travel arrangements and book the accommodation on your own. Do not make any arrangements until you receive the answer at your application (that is if you were selected or not), unless the cancellation will be possible till mid May! If you will be selected for the course, you will receive a code to get discounted rooms at Hotel “Timisoara” (single:67E/night, double 77E/night -breakfast and taxes included; availability not granted), but you can choose, of course, any other accommodation.
A few grants for travel and accommodation (200 E or 350 E) will be provided by ESPR for very-low-income countries, on a first-come first-served basis. You may apply for this grant by filling the dedicated section in the registration form. A proof of overall income is requested (Revenue tax declaration / Salary sheet / Proof of salaries /…). Be aware that participants have to pay everything in advance and the reimbursement will be made only after the end of the course; proof of costs will be necessary. Extra / particular support will be considered for applicats from Ukraine.

CoViD19/SARS-CoV2 PANDEMIC RELATED INFORMATION
There are no CoViD-related restriction any more in Romania! Anyhow, masks are still recommended indoors and in crowded areas and compulsory in hospitals. Course participants will have to wear mask (surgical or FFP2) during the hands-on sessions and are strongly encouraged to wear it during lectures, workshops, breaks and social activities.

How to arrive: Hotel Q is located at no more than 2 km from the Brasov Main Railway Station; Taxi is available. The Children Hospital is at 5 min. walk from the hotel. Airports: Sibiu (1.5 hours drive) and Bucharest-Otopeni (3 hours drive). Some hotels in Brasov have own shuttle to pick you up at the airport. A good option to arrive in Brasov is by train.

Propunerea unor modificari statutare a fost anuntata cu un an in urma. Datorita necesitatii alinierii la Statutul SRIM, a fost asteptata si definitivarea acestuia.

Noul Statut GRP prevede si existenta unui Cenzor.
Doar membrii GRP cu cotizatia platita vor putea vota.
